Examination measures limb circumference at standardized landmarks (every 4 cm above and below olecranon for arm; medial malleolus, mid-calf, mid-thigh for leg), tests Stemmer's sign at the base of the second toe or finger, palpates regional nodes, and assesses skin changes (hyperkeratosis, papillomatosis, lymphorrhea). Calibrated optoelectronic perometry or bioimpedance spectroscopy quantifies subclinical (Stage 0) and early Stage I disease before clinical swelling — a 10% volume increase from baseline meets International Society of Lymphology threshold. Doppler ultrasound rules out deep vein thrombosis and chronic venous insufficiency as alternative or contributing causes. Lymphedema is a chronic, lifelong condition once established but remains highly manageable with adherent treatment. Early-stage (ISL Stage 0-I) disease has the best prognosis: with prospective surveillance and prompt CDT, more than 70% of cases can be reversed or stabilized at baseline limb volume.
