Hereditary anemias such as thalassemia major and sickle cell disease have improving but still limited life expectancy without curative transplant or gene therapy; mild trait carriers have a normal lifespan. The decisive prognostic factors are speed of diagnosis, identification of the underlying cause, and adherence to repletion and surveillance.
A hematologist should be involved when anemia is severe (Hb under 8 g/dL without obvious cause), unresponsive to first-line therapy, pancytopenic, hemolytic, transfusion-dependent, suspected to be inherited, or accompanied by abnormal smear findings such as blasts or schistocytes.
