A hematologist should be involved when anemia is severe (Hb under 8 g/dL without obvious cause), unresponsive to first-line therapy, pancytopenic, hemolytic, transfusion-dependent, suspected to be inherited, or accompanied by abnormal smear findings such as blasts or schistocytes.
