The AAN/EFNS 2008 guideline recommends routine MRI for every patient with trigeminal neuralgia regardless of clinical confidence in a classical diagnosis. The natural course of trigeminal neuralgia is relapsing and remitting, with progressively shorter pain-free intervals over years.
